E-rate Form 470 Now Available
The new E-rate Form 470 for Funding Year 2015 is now available online via the USAC website. The Form 470 is the first form in the E-rate process.
IMPORTANT: As part of the E-rate Modernization Order, starting with the 2015 Funding Year, all E-rate forms must be filed online. Paper forms are no longer available.
Also, there are still three more E-rate workshops coming up, and there is space available in all of them:
November 24 – Scottsbluff Public Library
November 25 – Lincoln, Nebraska Library Commission
December 2 – Online, GoToWebinar
To register for any of these “The New E-rate: Preparing for Funding Year 2015 and Beyond” sessions, go to the Nebraska Library Commission’s Training & Events Calendar and search for ‘e-rate’.
Contact Christa Burns, 800-307-2665, 402-471-3107, if you have any questions or need any assistance with your E-rate application.
